Observation

Play yard for infant/toddler has larger rocks that pose choking hazard. Infant classroom has floor liner around vinyl that is sharp with exposed nails and sharp edges. Toddler classroom has wall beam board sticking out from wall that is sharp. Outdoor fence has broken brackets at bottom that are sharp. Pre-k classroom has heat vent in need of covering in restroom. Broken toys on playground and vents in classroom in need of covers. Toddler classroom has vinyl flooring torn in few places. Vinyl flooring needs secured to floor in several classrooms. Book shelfs and water coolers in need of being secured to walls. Flaking paint present on furniture, walls, and toys in need of disposing due to being broken. Toddler/preschool restrooms have glass lighting covers. Outdoor play area has exposed wires sticking up from ground and are frayed. Infant classroom has tape around safety foam on pole that is not cleanable.
